Some collective bargaining agreements have a probationary period which no contributions are required. Please verify with the Local Union and/or your employer for more information.
Once contributions are required and you have met 2 months out of the 4 consecutive months of 80 hours or more waiting period, the UFCW Trust will mail to you an enrollment form for you to complete and return. If you do not return the form within your enrollment period, you will automatically be deemed to “opt out” of coverage and will not be enrolled. You will have to wait until the annual open enrollment period or during any 30-day special enrollment window if you opt out of coverage initially.

There is no additional cost to you to enroll dependent(s). Dependents can be enrolled immediately when you become eligible for benefits.

The best document for their enrollment is a marriage certificate. However any legal document (such as a tax return) is sufficient.

There is no subsidized coverage for your spouse at Level 1. However, your spouse may qualify for benefits after you complete 1,200 hours of covered employment and an additional premium of $689.75. If elected, benefits for a spouse begin 60 days following the end of the month in which your 1,200th hour of eligible work occurs. Please contact the Trust Office for additional information.
Full family coverage would be provided under Levels 2 and 3 and there is no additional cost for your legally married spouse. However, if your spouse has benefits coverage available to them at their place of work and they opt not to enroll in their employer’s plan, there is a $100 per month charge for your spouse to enroll in the UFCW benefits plan. Please refer to your collective bargaining agreement to confirm your length of time with employment for Level 2 or 3.

You become eligible for Plan B of Dental Coverage once you have satisfied your employment requirements to have benefits under Level 2. Once you become a Level 3 Employee, you become eligible for Plan A of the Dental Coverage. Your spouse and dependent(s) become eligible for the same dental benefits once you are and will be automatically enrolled into dental if also enrolled under the Medical coverage.

If you work at least one day in a month and terminate employment that month, your coverage will terminate at the end of that month provided you worked your 80 hours or more in the previous month and are eligible for benefits. You will be offered COBRA Continuation at that time.
